The "Goofy's Epcot Guide" from 2004.

"Goofy's Epcot Guide" was a small guidebook about Epcot for children.  It had trivia questions for most of the attractions in Future World and countries in World Showcase, and once the final puzzle was correctly solved, the children could take the puzzle to one of a few designated places to claim a prize.
